I am writing as X’s Primary Worker, based with the Early Intervention Team. I have witnessed X’s transition from hospital to the placement provided by New Pathways in October 2008 and would like to commend New Pathways as a service provider.
During the transition period, New Pathways have maintained close links with all those involved in X’s care, with excellent communication and liaison. They have met with the agreed plan of care and sought to promote X’s independence and well-being. New Pathways have taken a holistic approach to X’s care and have continued to meet her physical, educational, social, mental health and leisure needs. New Pathways have facilitated contact with X’s mother, making X’s transition to care less stressful. The consistency of regular staff working with X has also ensured a good transition. My professional view is that the New Pathways placement has been paramount in X’s successful transition from hospital to ‘home’.
